Add troubleshooting steps to README #39

Description

Pre-reqs

What are you trying to do that currently feels hard or impossible?

Today if something is wrong with the configuration, the MCP server will not start but Gemini CLI will not report the reason why. This causes Gemini CLI to not be able to see the MCP tools which causes problems.

Suggested Solution(s)

We should update the README to include an FAQ and troubleshooting steps. We should tell the user to check /mcp list to confirm that the MCP server is online, and have some instructions on how to run Toolbox manually to get the error until Gemini CLI adds support.
